Description
Running superficially along the posteromedial calf, the great saphenous vein tracks anterior to the medial malleolus before ascending toward the medial knee and thigh, while the small saphenous vein begins posterior to the lateral malleolus and climbs in the midline of the posterior leg toward the popliteal fossa. Perforating veins bridge the superficial network to the deep system, communicating with the posterior tibial and fibular (peroneal) veins that accompany their corresponding arteries in the deep posterior compartment. Proximally, deep venous channels converge toward the popliteal vein behind the knee, with tributaries arranged around the gastrocnemius and soleus contours for orientation. Valve-bearing segments are suggested along the longitudinal trunks. Clear asymmetry stands out. Posterior lower-leg venous anatomy matters because it maps directly onto varicose vein patterns and the pathways of reflux, most often involving the small saphenous vein or incompetent perforators that channel high-pressure flow from the deep veins into superficial tributaries.
