Seen from the lateral aspect, the cerebellar hemisphere dominates the field, with tightly packed folia arranged into lobules separated by deeper fissures. Superiorly, the tentorial surface curves toward the vermis at the midline, while inferiorly the hemisphere slopes toward the cerebellar tonsil region and the inferior surface bordering the foramen magnum. Anteriorly, the cerebellar peduncle region sits nearest the brainstem, where the middle cerebellar peduncle (brachium pontis) forms the broadest connection, with the superior peduncle projecting rostrally and the inferior peduncle coursing caudally. A lateral cerebellar perspective is the teaching workhorse for cerebrocerebellar circuitry because it emphasizes the hemispheric cortex and its deep relationships to the peduncles that carry pontocerebellar input and cerebellothalamic output. This is the view you want when discussing ipsilateral limb ataxia from hemispheric lesions, or when mapping posterior fossa mass effect that can distort the peduncles and fourth ventricle region. Surgical planning for a retrosigmoid or far-lateral approach also benefits from clear recognition of the hemisphere’s inferior pole and the tonsillar contour. Small differences in fissure depth matter.
