Oriented from an anterior perspective, the cerebellum fills the posterior cranial fossa as paired cerebellar hemispheres flanking the midline vermis. Superiorly, the hemispheric surfaces curve toward the tentorial aspect, while inferiorly the contours slope toward the cerebellar tonsils near the foramen magnum. At the anterolateral margins, the cerebellar peduncles form the principal connections to the brainstem, with the middle cerebellar peduncle positioned lateral to the superior peduncle and superior to the inferior peduncle as the fibers converge toward the pons and medulla. Midline. Clear separation. An anterior cerebellar view is the one you reach when you mentally remove the brainstem and open the fourth ventricle, so it is the best orientation for teaching the cerebellar contribution to the roof of the ventricle and the relationship of peduncles to brainstem entry zones. That relationship matters in posterior fossa surgery and imaging interpretation, where mass effect from a hemispheric lesion or cerebellar edema can compress the fourth ventricle and obstruct CSF flow, producing acute hydrocephalus and tonsillar herniation. It also maps cleanly to clinical localization, since infarcts in the PICA or SCA territories often present with ipsilateral limb ataxia and dysmetria that correlate with hemisphere involvement rather than vermian truncal ataxia. Ideal use cases include neuroanatomy and neuroscience curricula covering the metencephalon and hindbrain connectivity, neurosurgical atlases discussing approaches around the cerebellopontine angle and cerebellar peduncles, and radiology teaching files that pair gross anatomy with axial and sagittal MRI of posterior fossa crowding.
