Description
Rendered in lateral profile, the mentalis occupies the anterior mandible at the chin (mentum), arising from the incisive fossa of the mandibular body inferior to the incisor roots and coursing inferiorly to insert into the dermis of the chin and the soft tissues of the lower lip (labium inferius). Its fibers sit deep to the subcutaneous fat pad and blend superiorly with the orbicularis oris and adjacent depressor labii inferioris, while lying anterior to the mandibular symphysis and inferior to the vermilion border. Posterior to this focal muscular anatomy, the illustration situates the region against the bony contour of the mandible, dentition, maxilla, and the temporomandibular joint with the condyle seated in the mandibular fossa. Orientation is unambiguous. Functionally, the mentalis elevates and protrudes the lower lip and wrinkles the chin skin, a motion that becomes obvious in lip incompetence when patients recruit it to achieve oral seal. Overactivity contributes to the pebbled chin appearance and can deepen the mentolabial sulcus, which is why targeted chemodenervation requires careful placement to avoid lower lip asymmetry from spread into the depressor labii inferioris or orbicularis oris. The lateral perspective also helps you think in layers when planning submental or perioral incisions, and when protecting the mental nerve as it exits the mental foramen to supply sensation to the chin and lower lip.
