Description
Seen in left lateral profile beneath an intact cutaneous layer, the mentalis forms a compact, vertically oriented muscle pad over the anterior mandible at the level of the chin, just inferior to the lower lip and anterior to the pogonion. Its fibers arise from the incisive fossa of the mandible and run inferiorly toward the dermis, lying medial to the depressor anguli oris and anterior to the depressor labii inferioris at the perioral margin. Posteriorly, the masseter occupies the lateral mandibular ramus, while the platysma sheets inferiorly across the anterolateral neck; superiorly, orbicularis oris encircles the oral aperture and blends with adjacent mimetic musculature. Fiber direction is explicit. This lateral, subcutaneous treatment matters because mentalis function is inseparable from the skin. Contraction elevates and protrudes the lower lip and wrinkles the chin, the classic “doubt” or “pouting” expression, and it also helps maintain lower incisor competence, a key teaching point when discussing lip seal and drooling in facial nerve dysfunction. In aesthetic practice, mentalis hyperactivity contributes to a cobblestoned chin and can worsen the labiomental fold, so accurate localization guides botulinum toxin placement and helps avoid complications such as lower lip ptosis from diffusion into depressor labii inferioris.
