Description
Arising from the anterior surface of the mandible at the incisive fossa, the mentalis appears as a compact paired muscle belly positioned inferior to the lower lip and superior to the mental prominence of the chin. In lateral profile it lies anterior to the mandibular symphysis and inferior to the orbicularis oris, with its fibers running superomedially toward the skin of the chin where they blend with the dermis. Posteriorly, the mandibular body and alveolar process with the lower incisors and canine provide a hard-tissue reference, while superiorly the maxilla, zygomatic arch, and nasal cavity establish facial proportions in true lateral view. Mentalis anatomy matters because it is the primary chin elevator and lower-lip protruder, the small motor that creates the classic pout and helps achieve lip seal during speech and drinking. Subtle asymmetry or hyperactivity can become obvious after dental procedures, mandibular trauma, or orthognathic surgery, and mentalis strain is a common compensatory pattern in patients with lip incompetence. For injectors, this lateral relationship to the mandibular symphysis and lower lip is a landmark when treating a pebbled chin (mentalis overcontraction) with botulinum toxin while avoiding excessive lower-lip weakness.
