Mentalis Muscle, Anterior View

Description

Centered on the anterior chin, the mentalis muscle appears as a paired, conical muscle mass arising from the incisive fossa of the mandible inferior to the incisor teeth and coursing inferiorly to insert into the dermis of the chin. Its fibers sit deep to the skin and superficial to the mandibular symphysis, positioned inferior to the lower lip and medial to the depressor anguli oris on either side. Superiorly, the mentalis blends with the orbicularis oris at the level of the labiomental sulcus, while more laterally the depressor labii inferioris tracks down toward the lower lip. Surrounding facial musculature such as the zygomaticus major and minor and the orbicularis oculi may be visible for orientation, with the platysma extending inferiorly over the anterior neck. For clinicians, this is the landmark muscle for chin elevation and lower-lip protrusion, and it is the structure that dimples the skin when it contracts. Weakness or denervation of mentalis, sometimes seen after mandibular trauma or iatrogenic injury to branches of the facial nerve during lower-face procedures, produces a flattened chin and reduced labiomental crease. A clean anterior rendering also supports teaching of the mental nerve territory, since the mental foramen lies lateral to the mentalis region and local anesthetic blocks can alter lower-lip competence and the appearance of chin tone.
