Description
Positioned between the posterior margin of the foramen magnum and the posterior arch of the atlas (C1), the posterior atlantooccipital membrane spans laterally toward the transverse processes, forming a fibrous roof over the vertebral artery groove. From this lateral perspective, the occipital bone sits superior to C1, with the mandibular ramus and temporomandibular joint anteriorly and the cervical spinous processes descending inferiorly. Cranial sutures, the temporal bone, and the dental arches provide bony context while keeping attention on the thin sheetlike ligament at the craniocervical junction. A clean landmark. Clinically, this membrane matters because the vertebral artery and suboccipital nerve (C1 dorsal ramus) pass immediately deep to it as they course from the transverse foramen of C1 toward the dura, so an overly aggressive posterior exposure can create arterial injury or postoperative occipital neuralgia. Its lateral free edge often forms the arcuate foramen (ponticulus posticus) when ossified, a variant that can tether the artery and complicate C1 screw placement or manipulation during upper cervical mobilization. For teaching, the relationship between the posterior atlantooccipital membrane and the posterior atlantoaxial membrane helps learners distinguish the occiput to C1 interval from the C1 to C2 interval in dissection and cross sectional imaging.
