Description
Seen in lateral sagittal section, the tela choroidea forms a thin, vascular double layer of pia mater spanning the roof of the third ventricle and extending posteriorly toward the region of the pineal recess, where it relates closely to the internal cerebral veins. Superior to it lie the thalamic structures and the fornix, while the hypothalamus sits anteroinferiorly, bordering the ventricular cavity where the choroid plexus attaches along the choroidal fissure. Posteriorly and inferiorly, the midbrain and upper brainstem provide a deep landmark, with adjacent fiber pathways coursing anteroposteriorly in the diencephalic region. Small in thickness, large in consequence. This lateral perspective matters when you need to explain how CSF-producing choroid plexus is suspended from pia and invaginates into the ventricular system without creating a true open communication between ventricle and subarachnoid space. The tela choroidea and its venous relationships form part of the operative and radiologic map for intraventricular pathology, including colloid cysts near the foramen of Monro, third-ventricular tumors, and cystic lesions that distort the choroidal fissure and internal cerebral veins. It also clarifies why infection or hemorrhage can track along ventricular surfaces while the pia-based folds remain the substrate for plexus attachment.
