Description
Seen in lateral profile, the transverse facial vein courses horizontally across the cheek, superficial to the masseter and closely accompanying the transverse facial artery as it emerges from the parotid region just anterior to the auricle. It lies inferior to the zygomatic arch and posterolateral to the buccinator, receiving small buccal and parotid tributaries before turning posteriorly toward the venous channels around the gland. Branches of the facial nerve (parotid plexus) can be appreciated fanning anteriorly from within the parotid gland, with the sternocleidomastoid and platysma forming key superficial landmarks of the lateral neck. Orientation is clear. For parotid surgery this relationship matters. During a superficial parotidectomy or exposure of the masseteric region, the transverse facial vein is a consistent bleeding source that crosses the operative field near the zygomatic arch while the facial nerve branches, just deep and slightly inferior in the parotid tissue, remain at risk of traction or thermal injury. The vessel’s proximity to the parotid duct and the buccal branch of the facial nerve also makes it a practical landmark when teaching safe dissection planes.
