Running horizontally across the lateral cheek, the transverse facial vein courses superficial to the masseter and buccinator, typically emerging from the region of the parotid gland to join venous channels leading into the retromandibular vein. Superiorly, the zygomatic arch and temporalis define the upper boundary of the field, while the mandible and the angle of the jaw sit inferior and posterior to the main venous pathway. Red arterial companions are suggested in parallel, and yellow nerve branches trace the expected routes of the facial nerve within or adjacent to parotid tissue and across the midface. Bony landmarks of the maxilla and mandible anchor the soft tissue relationships. Appreciating this vein’s position relative to the parotid, masseter, and facial nerve matters in lateral facial surgery. The transverse facial vein can be encountered during parotidectomy, facelift (rhytidectomy) dissection, and repair of zygomaticomaxillary complex injuries, where venous bleeding and proximity to buccal and zygomatic branches of the facial nerve complicate the plane. Venous drainage into the retromandibular pathway also provides a clean teaching bridge between superficial facial venous networks and the deep venous system of the parotid region.
