Description
Running along the lateral border of the plantar forefoot, the abductor digiti minimi (abductor digiti quinti) is seen beneath the skin as a slender red muscle belly arising from the lateral process of the calcaneal tuberosity and the plantar aponeurosis, then coursing anteriorly toward the base of the proximal phalanx of the fifth toe. Deep and slightly medial to it, the quadratus plantae and the flexor digitorum brevis are suggested by their relative bulk and tendon trajectories, while the long flexor tendons (flexor hallucis longus and flexor digitorum longus) pass posterior to the medial malleolus and continue distally within the sole. Posteriorly, the calcaneal (Achilles) tendon inserts on the calcaneal tuberosity superior to the plantar musculature, and the calcaneus, talus, cuboid, and lateral metatarsals define the bony framework beneath the soft tissues. Lateral ankle stabilizers and subtalar region ligaments anchor the hindfoot, framing the muscle planes that support the lateral longitudinal arch. For clinicians, this lateral plantar view matters because the abductor digiti minimi lies directly in the corridor used to localize the lateral plantar nerve and vessels, and it is the most commonly affected muscle in lateral plantar nerve entrapment (Baxter neuropathy) with denervation edema and atrophy on MRI. Pain at the medial calcaneal tubercle often gets labeled plantar fasciitis, but selective tenderness and weakness in fifth-toe abduction can steer the differential toward nerve compression deep to the abductor.
