Description
Seen from the lateral aspect of the male foot and distal leg, the adductor digiti minimi (adductor digiti quinti) lies on the plantar surface along the lateral border of the sole, deep to the plantar aponeurosis and closely related to the fifth metatarsal and proximal phalanx of the fifth toe. Posteriorly, the calcaneus and Achilles tendon anchor the hindfoot, while the talus sits superior to the calcaneus within the ankle mortise formed by the distal tibia and fibula. Distally, the long flexor and extensor tendons track toward the phalanges, framing the small intrinsic muscle that pulls the fifth digit toward the midline of the forefoot. Small, deep anatomy. Big teaching value. Lateral presentation of the adductor digiti minimi matters when you are explaining intrinsic plantar layering and why lateral column pain is not always “just peroneals.” The muscle’s line of pull toward the base of the proximal phalanx helps stabilize the fifth ray during stance and push-off, and weakness or irritation can accompany cavovarus mechanics, tailor’s bunion (bunionette) deformity, and postoperative stiffness after fifth metatarsal osteotomy. It also provides a clean reference for differentiating intrinsic muscle bulk from the more superficial abductor digiti minimi and from tendinous structures crossing the lateral ankle.
