Description
Seen from the lateral aspect of a male upper limb, the brachialis occupies the anterior surface of the distal half of the humerus, deep to the biceps brachii and tapering inferiorly toward the elbow. Its broad muscular belly lies medial to the brachioradialis at the lateral supracondylar region and crosses the anterior elbow joint to insert on the ulnar tuberosity and coronoid process, just distal to the trochlear notch. The humerus, proximal radius and ulna, and the articulating surfaces of the elbow are visible in relationship to the muscle’s line of pull. Radius and ulna orientation clarifies the lateral (radial) versus medial (ulnar) sides as the forearm approaches the carpus and hand. For elbow flexion teaching, brachialis is the cleanest example of a single joint, high torque flexor because it acts on the ulna and remains a prime mover in both pronation and supination, unlike biceps brachii. That distinction matters clinically when differentiating musculocutaneous nerve injury from radial or median neuropathies during manual muscle testing, since brachialis (C5 to C6) can preserve elbow flexion even when forearm supination is weak. It also frames anterior elbow approaches, where the biceps tendon and lacertus fibrosus are superficial landmarks while brachialis forms the floor overlying the brachial artery and median nerve before their bifurcation and passage into the forearm. A deep landmark.
