Description
Deep to the biceps brachii, the brachialis forms the broad anterior bulk of the distal arm (brachium), draping across the anterior surface of the humeral shaft and converging inferiorly toward its insertion on the ulnar tuberosity and coronoid process. Medially it approaches the intermuscular septum and the medial epicondylar region, while laterally it blends toward the lateral supracondylar ridge and lies adjacent to the brachioradialis origin. In a posteriorly oriented, skeleton-referenced layout, the brachialis is understood by its relationship to the humerus and ulna even when the scapulae, vertebral column, and posterior arm contours serve as the main spatial frame. Brachialis is the primary elbow flexor across forearm positions because it acts on the ulna, not the radius, so its contribution remains steady during pronation and supination when biceps brachii torque varies. This is the muscle you palpate in the distal anterior arm just medial and lateral to the biceps tendon during resisted flexion, and it becomes a practical landmark when discussing musculocutaneous nerve supply (C5 to C6) and pain patterns after anterior elbow strain. Clinically, it is often injured in eccentric loading and is directly relevant when teaching why elbow flexion can remain strong even with distal biceps tendon rupture.
