Description
Anterior arm anatomy is centered on the brachialis, a thick fusiform muscle lying deep to biceps brachii and spanning the distal half of the humeral shaft to the proximal ulna. Its fibers arise broadly from the anterior surface of the humerus and adjacent intermuscular septa, then converge distally to insert on the ulnar tuberosity and coronoid process, immediately inferior to the trochlear notch. Medial and lateral borders of the muscle hug the humeral cortex, placing it directly anterior to the elbow joint capsule and just proximal to the ulna as the forearm begins. Bone landmarks matter here. Brachialis is the workhorse elbow flexor because it crosses only the humeroulnar joint, so it produces flexion regardless of forearm pronation or supination, unlike biceps brachii which is mechanically advantaged in supination. That distinction becomes clinically relevant when assessing elbow flexion weakness after musculocutaneous nerve injury, or when differentiating biceps tendon rupture (loss of supination strength, Popeye deformity) from a more proximal anterior compartment lesion where brachialis involvement reduces pure flexion torque. Surgeons approaching the distal humerus or anterior elbow also use the brachialis as a landmark and a protective layer over the joint capsule, while remaining alert to its lateral split where the radial nerve passes from posterior to anterior compartments.
