Description
Seen from a lateral perspective in the male upper arm, the brachialis occupies the anterior compartment deep to the biceps brachii, arising broadly from the distal half of the humeral shaft. Its muscle belly wraps anterolaterally toward the elbow, passing anterior to the joint and narrowing into a stout tendon that inserts on the ulnar tuberosity and adjacent coronoid process. Laterally, the brachialis lies close to the brachioradialis and extensor carpi radialis longus at the lateral supracondylar ridge, while remaining anterior to the triceps brachii and the posterior humerus. Functionally, brachialis is the workhorse elbow flexor because it crosses only the humeroulnar joint, so its contribution remains consistent in both pronation and supination when biceps mechanics change. A good lateral view clarifies why distal humeral fractures and surgical exposure around the lateral column can compromise elbow flexion, and it helps locate where the musculocutaneous nerve typically supplies the muscle on its anterior surface, with frequent accessory innervation from the radial nerve near the lateral portion. This anatomy also frames a common teaching point: swelling or hematoma deep to biceps can mask brachialis injury after blunt trauma, yet flexion weakness persists even when supination strength is preserved.
