Description
Centered on the anterior arm, the brachialis occupies the distal half of the humeral shaft and fans broadly toward the elbow, lying deep to the biceps brachii and crossing the joint anteriorly. Its muscle belly sits anterior to the humerus and proximal to the ulna, then narrows distally to a tendon that inserts on the ulnar tuberosity and coronoid process, medial to the radial tuberosity where the biceps attaches. Lateral to this insertion, the radius articulates with the capitulum, while the ulna forms the trochlear notch and medial column of the hinge. The relationship is straightforward. Brachialis is the floor under biceps. Because brachialis is the primary elbow flexor regardless of forearm pronation or supination, an anterior perspective helps separate true elbow flexion mechanics from the biceps’ dual role in flexion and supination. This view also supports clinical teaching around distal biceps tendon rupture: patients may retain elbow flexion strength through an intact brachialis yet lose forearm supination power, a pattern that matches the different insertions on radius versus ulna.
