Description
Medial anatomy of a flexed knee is presented with the distal femur superiorly and the proximal tibia inferiorly, their medial condylar and plateau surfaces aligned in the femorotibial articulation. Along the medial side, the tibial collateral ligament (medial collateral ligament) spans from the medial femoral epicondyle to the medial tibial shaft, crossing the joint line as a broad, longitudinal band. Deep to it, the capsular thickening consistent with the deep medial collateral complex blends with the medial joint capsule and would lie adjacent to the medial meniscus at the level of the tibial plateau. Patella and patellofemoral contours are out of plane in this strict medial perspective. Flexion is not a cosmetic choice. Bending the knee tensions different portions of the medial collateral ligament and exposes its long tibial attachment, a point that matters when you teach valgus stability and why grade II to III MCL injuries can remain symptomatic along the distal tibial insertion. This view also helps distinguish the superficial MCL from capsular structures that become relevant during medial arthrotomy and during repair or reconstruction for combined MCL and medial meniscal pathology.
