Description
Posterior to the distal femur and proximal tibia, the flexed femorotibial joint opens the intercondylar notch and brings the posterior cruciate ligament (PCL) into clear view as it runs from the lateral surface of the medial femoral condyle to the posterior intercondylar area of the tibia. Medial and lateral collateral ligaments sit more peripherally, spanning from the femoral epicondyles toward the tibia and fibula, framing the posterior capsule. The menisci occupy the tibial plateau between condyles, while the popliteal tendon and the oblique popliteal ligament typically cross the posterolateral and posteromedial aspects of the capsule, respectively. Patellofemoral structures are largely out of the field from this posterior perspective. Flexion is the position that best separates the cruciate ligaments functionally and visually, and it also mirrors how clinicians interrogate stability at the bedside. The posterior drawer test and posterior sag sign assess PCL integrity, and this view makes it easier to teach why the PCL is the primary restraint to posterior translation of the tibia relative to the femur at higher degrees of flexion. Arthroscopically and in PCL reconstruction, orientation to the intercondylar roof and tibial PCL footprint matters, and posterolateral corner injury patterns often coexist, complicating surgical planning. A high-stakes ligament.
