Description
Anteriorly, the flexed knee is opened up to emphasize the femorotibial and patellofemoral articulations, with the distal femoral condyles sitting superior to the tibial plateau and the patella positioned anterior to the distal femur within the extensor mechanism. The medial (tibial) collateral ligament runs along the medial joint line from the medial femoral epicondyle toward the proximal tibia, while the lateral (fibular) collateral ligament spans from the lateral femoral epicondyle to the fibular head on the lateral side. Within the intercondylar region, the anterior cruciate ligament courses from the anterior intercondylar area of the tibia to the posteromedial aspect of the lateral femoral condyle, and the posterior cruciate ligament runs from the posterior intercondylar tibia to the anterolateral aspect of the medial femoral condyle. Clear orientation. Even in flexion. Flexion is where ligament function becomes easier to teach and easier to injure: the ACL tightens and resists anterior translation of the tibia, while the PCL resists posterior translation, and both guide femoral rollback during knee bending. This perspective maps directly to common exam maneuvers and injury patterns, including ACL rupture from noncontact pivoting, PCL injury in dashboard trauma, and valgus stress causing MCL sprain, all framed against the anterior access surgeons use during arthroscopy and ACL reconstruction.
