Description
Running along the medial distal thigh, the descending genicular artery branches from the femoral artery just proximal to the adductor hiatus and courses inferiorly toward the medial aspect of the knee. Its saphenous branch tracks anterior to the adductor magnus and deep to the sartorius, then continues with the saphenous nerve toward the anteromedial proximal tibia, where it contributes to the periarticular genicular anastomosis. Adjacent venous channels accompany the arterial segments, shown in blue, while the femur lies superiorly with the proximal tibia entering the frame inferiorly. Key muscular neighbors include vastus medialis anterolaterally, the pes anserinus tendons at the medial tibia, and the posterior hamstring mass more dorsally. Medial knee vascular anatomy matters because the descending genicular artery is a primary pedicle for medial femoral condyle and supracondylar region perfusion, and its branches intersect surgical corridors used around the adductor canal. Injury or ligation can compromise collateral flow to the knee when the superficial femoral or popliteal artery is diseased, and the saphenous branch can be a source of persistent bleeding during medial approaches that split the sartorial fascia. It is a small vessel with big consequences.
