Description

Seen from the medial aspect of the knee, the descending genicular vein tracks distally along the medial distal femur toward the medial tibial condyle, running in close association with the descending genicular artery and its saphenous branch as they approach the pes anserinus region. The vessel sits superficial to the medial femoral condyle and medial joint capsule, then communicates with the great saphenous vein and adjacent superficial tributaries along the medial border of the leg. Superiorly the distal femoral shaft forms the deep bony backdrop, while inferiorly the proximal tibia and the intercondylar region anchor the vascular arc around the joint line. Medial knee vascular anatomy matters when you are trying to stay out of trouble during approaches that use the anteromedial window or work along the medial retinaculum and proximal tibia. Injury to the descending genicular vessels can contribute to postoperative hematoma or compromise local perfusion, and their periarticular network becomes clinically relevant in medial knee trauma and during flap planning that incorporates the saphenous branch territory.
