Description

Medial exposure of the male hip and proximal thigh brings the adductor compartment into the foreground, with adductor longus lying superficial and anterior, gracilis running as a long medial strap, and adductor magnus broad and posterior along the linea aspera of the femur. Superiorly, the pubic body and inferior pubic ramus frame the muscular origins, while the acetabulum and femoral head sit lateral to the adductor origins at the hip joint. Anteromedially, sartorius and vastus medialis define the transition toward the anterior compartment, and pectineus occupies the upper medial thigh between the psoas tendon region and adductor longus. White tendinous sheets and aponeuroses mark the pubic attachments and the adductor insertions along the femoral shaft. Dense anatomy. This medial angle matters because it clarifies the layered arrangement that clinicians palpate and image when evaluating groin pain, and it separates the more anterior adductor longus and pectineus from the deeper adductor brevis and the posterior fibers of adductor magnus. The relationship of the adductor longus origin to the pubic symphysis and anterior pelvis is a common site of athletic pubalgia and adductor tendinopathy, while the interval between sartorius and vastus medialis leads into the adductor canal where the femoral artery becomes difficult to access without an accurate mental map. A medial view also supports teaching of obturator nerve distribution to the adductors and how weakness or denervation changes hip adduction mechanics.
