Description
Seen from the medial aspect of a male thigh, the femoral artery descends in the femoral triangle and continues into the adductor canal, where it lies anteromedial to the adductor longus and then deep to sartorius as it approaches the adductor hiatus. Proximal branches such as the profunda femoris (deep artery of the thigh) and its medial circumflex femoral contribution are suggested near the posteromedial femur, while the vessel’s course toward the popliteal artery is traced distally as it passes posteriorly through the adductor magnus. Companion deep veins, including the femoral vein, track close to the artery, typically positioned posterolateral proximally before shifting relationship along the canal. Spatial relationships matter here. Medial exposure highlights the corridor surgeons use for vascular control and bypass targets, because the superficial femoral artery is approached through the interval between sartorius and the adductor group, and its transition through the adductor hiatus explains why distal pulses can be present even when thigh pain suggests exertional ischemia. This perspective also supports teaching around iatrogenic injury patterns, for example during adductor canal blocks, where local anesthetic spread can reach the saphenous nerve and nerve to vastus medialis while the femoral vessels remain the structures to avoid. The adductor canal is the landmark.
