Description
Seen from the plantar aspect, the deep plantar arch curves transversely across the midfoot, coursing from the lateral plantar artery toward its anastomosis with the deep plantar branch of the dorsalis pedis artery. Metatarsal bases and distal tarsal bones form the skeletal backdrop, with the arch lying deep to the plantar aponeurosis and spanning between the region of the fifth metatarsal laterally and the first intermetatarsal space medially. From the convexity of the arch, plantar metatarsal arteries run anteriorly (distally) toward the toes, while accompanying venae comitantes and plantar venous channels track alongside in blue. Bony landmarks orient you quickly. This plantar perspective matters because it clarifies the vascular cross-connection between the anterior and posterior tibial systems at the level of the tarsometatarsal joints, a junction that becomes clinically concrete during forefoot and midfoot surgery. Pedicle planning for plantar-based flaps, metatarsal osteotomies, and Lisfranc fixation all risk iatrogenic injury to the deep plantar arch and proximal plantar metatarsal vessels if dissection drifts too deep between the metatarsal bases. Ischemic forefoot pain in diabetes and peripheral arterial disease also maps onto this anatomy, where reduced inflow through the lateral plantar artery can compromise digital perfusion despite a palpable dorsalis pedis. Collateral pathways are not uniform.
