Posterior Perspective of the Ileocolic Artery

Description

Seen from a posterior angle in an adult male abdomen, the superior mesenteric artery gives rise to the ileocolic artery as it courses inferiorly and to the right toward the ileocecal junction. Near the terminal ileum and cecum, the vessel divides into ileal and colic branches, with an anterior and posterior cecal artery typically curving around the cecum and an appendicular artery extending toward the vermiform appendix. Posteriorly, the right kidney and great vessels, the abdominal aorta and inferior vena cava, provide a midline reference while the mesentery and adjacent small and large bowel frame the branching pattern. Posterior orientation matters for surgeons because the ileocolic pedicle is commonly approached and ligated during right hemicolectomy, ileocecal resection, and appendectomy, where the relationship of the ileocolic artery to the superior mesenteric vein and the retroperitoneum guides safe dissection. Variants are common. A high origin, early branching, or a prominent appendicular artery can influence how far proximally a surgeon takes the vascular tie to achieve oncologic lymphadenectomy while preserving perfusion to the terminal ileum and remaining colon.
