Description
Emerging from the ulnar artery in the proximal forearm, the posterior ulnar recurrent artery courses superiorly and posteriorly toward the elbow, tracking along the medial side of the ulna before curving behind the medial epicondyle. In a posterior perspective with the limb pronated, the vessel is seen deep to the superficial venous plane and in close relationship to the olecranon, trochlear notch, and the distal humerus. Branches contribute to the periarticular network around the elbow, where the recurrent vessel meets descending collateral flow from the brachial artery to complete the arterial anastomosis. Small arteriolar offshoots are suggested as they disperse into the posterior capsular and muscular envelopes. Posterior elbow perfusion is often discussed abstractly, but this recurrent artery makes it concrete. Trauma, posterior approaches for olecranon fixation, and medial exposures near the cubital tunnel bring instruments into the same corridor where the artery ascends, and its proximity to the ulnar nerve as it passes posterior to the medial epicondyle explains why bleeding and neuropathic symptoms can present together. Collateral circulation around the elbow also matters when the brachial artery is temporarily clamped or injured, because retrograde flow through the ulnar recurrent system can help preserve distal perfusion.
