Description
Posterior pelvic anatomy is rendered with emphasis on the pudendal venous drainage in an adult male. The internal pudendal vein courses deep to the gluteus maximus and adjacent to the internal pudendal artery, exiting the pelvis inferior to the piriformis, wrapping around the ischial spine near the sacrospinous ligament, then entering the perineum through the lesser sciatic foramen to run in Alcock’s canal along the lateral wall of the ischioanal fossa. Superficial tributaries from the external genitalia contribute via the superficial external pudendal vein toward the great saphenous and femoral veins anteriorly, while deeper venous channels from the penis and perineum communicate with the prostatic and vesical plexuses. Bony landmarks, including the ilium, ischium, and proximal femur, frame the vascular course. Posterior perspective matters because it aligns the vessel with the landmarks used in pelvic and perineal surgery. The ischial spine, sacrospinous ligament, and pudendal canal are the key reference points for pudendal nerve block and for approaches to pudendal nerve entrapment, and the accompanying veins are often the first structures encountered and a common source of bleeding in the ischioanal fossa. This is also the corridor where internal pudendal vessel injury can complicate drainage of perianal abscesses or fistula surgery. Small space. High stakes.
