Description
Posterior thoracic wall anatomy is presented as an upright male rib cage centered on the vertebral column, with the thoracic vertebrae continuing inferiorly into the lumbar spine and the superior sacrum. Paired ribs sweep laterally from the costovertebral and costotransverse joints to form the posterior arcs of the thoracic cage, their heads and tubercles nested against the vertebral bodies and transverse processes. Subtle rib-to-rib differences are legible, including the shorter, more horizontal superior ribs and the longer, more obliquely oriented mid and inferior ribs as they course anterolaterally out of frame. Select ribs are color-highlighted to help track individual levels across the posterior curvature. Costovertebral relationships matter when you are localizing pain and planning access. Posterior rib fractures often occur just lateral to the costotransverse region, and even small displacement can narrow the intercostal space and threaten the intercostal neurovascular bundle that runs along the inferior rib margin. This posterior perspective also supports level counting for thoracic epidural or paravertebral blocks, where the rib angle and adjacent spinous processes are practical landmarks, and it clarifies why inflammatory arthropathy or septic involvement of costovertebral joints can refer pain along a rib.
