Description

Seen from posterior in the male neck, the paired thyroid lobes flank the upper trachea and lower larynx, joined across the midline by the isthmus. Blue venous channels course on the posterior aspect of the gland, collecting from the capsular plexus and running inferiorly toward the thoracic inlet, with smaller venules interconnecting across the midline. Small parathyroid glands sit on the posterior surface of each lobe, typically near the junction of the superior and middle thirds, closely related to the posterior thyroid capsule. Spatially, the thyroid veins lie deep to the infrahyoid (strap) musculature and anterolateral to the tracheoesophageal groove. A posterior perspective matters because it matches what the surgeon and trainee must mentally reconstruct during thyroidectomy or parathyroid exploration, when venous bleeding often comes from the posterior capsular plexus and the inferior thyroid veins as the gland is mobilized medially. Control of these veins is not just hemostasis, it also affects traction on the recurrent laryngeal nerve, which ascends in the tracheoesophageal groove immediately posterior and medial to the thyroid lobe. This is a common source of postoperative hoarseness. The view also supports teaching of why parathyroid preservation depends on respecting the posterior capsule and its vascular relationships.
