Description
Arising at the cervicothoracic junction, the first thoracic vertebra (T1) sits inferior to C7 and superior to T2, centered on the posterior midline of the spinal column in a male. From a posterior perspective, the midline spinous processes form a stacked ridge, with T1 identifiable by its long, inferiorly sloping spinous process and the broader laminae leading laterally to the transverse processes. Just lateral to the vertebral arch, the superior articular processes face posteromedially while the inferior articular processes angle anterolaterally, defining the transition from cervical to thoracic facet orientation. Adjacent cervical, thoracic, lumbar vertebrae, plus the sacrum and coccyx, provide level-to-level context. T1 matters because it marks the first vertebral level with costal facets for rib articulation, anchoring the first rib and contributing to the thoracic inlet.
