Positioned at the cervicothoracic junction, the first thoracic vertebra (T1) is presented from an anterior perspective within the full vertebral column, from C1 inferiorly to the sacrum and coccyx. From this angle, the vertebral body of T1 sits inferior to C7 and superior to T2, with the pedicles projecting posterolaterally toward the vertebral arch. Along the superolateral margins of the body, the superior costal facets (foveae costales superiores) mark the articulation site for the head of the first rib, with the inferior costal facet for the second rib typically placed more caudally on the body. Orientation stays clear in anterior view: vertebral bodies align medially, transverse processes flare laterally, and spinous processes remain posterior and largely obscured. T1 matters because it is a key landmark for rib counting and for interpreting imaging around the thoracic inlet. On radiographs and CT, the first rib’s relationship to the T1 superior costal facet helps confirm level identification when cervicothoracic anatomy is ambiguous, and that precision carries into procedures such as upper thoracic epidural placement or posterior instrumentation. This segment also frames clinical problems that cluster at the thoracic outlet, where first rib position and T1 level localization intersect with brachial plexus and subclavian vessel pathways.
