Description
Running obliquely along the posterolateral neck, the sternocleidomastoideus (sternocleidomastoid) appears as a superficial strap muscle under the skin, coursing from the mastoid process and lateral superior nuchal line inferoanteriorly toward its sternal head on the manubrium and its cleido portion on the medial clavicle. From this posterior angle, its posterior border helps define the lateral contour of the neck as it approaches the mastoid attachment, which becomes partially obscured by the splenius capitis and semispinalis capitis deep to it. Posterior to the sternocleidomastoideus, the trapezius forms the broad superficial sheet over the upper back, with the nuchal ligament and cervical spinous processes marking the midline and the upper erector spinae columns visible inferiorly. This posterior perspective matters because the posterior border of the sternocleidomastoideus is the anterior boundary of the posterior cervical triangle, a working space where the spinal accessory nerve (CN XI) crosses superficially toward the trapezius and is prone to iatrogenic injury during lymph node biopsy. Muscular asymmetry or spasm here is also the clinical signature of torticollis, where unilateral sternocleidomastoid shortening rotates the face contralaterally and tilts the head ipsilaterally, while deeper extensors such as splenius capitis can mask or perpetuate the deformity.
