Description
Running obliquely along the posterolateral contour of the male neck, the sternocleidomastoideus muscle can be followed from its superior attachment at the mastoid process and lateral superior nuchal line toward its inferior heads on the manubrium sterni and medial clavicle, even as the viewpoint favors the posterior neck. Its posterior border forms a distinct ridge lateral to the cervical spinous processes, with the trapezius occupying the more posterior midline and the splenius capitis and levator scapulae filling the deep posterolateral neck. Inferiorly, the upper thoracic vertebrae and the medial margins of the scapulae anchor the composition, keeping the relationship between cervical musculature and the shoulder girdle clear. Bone landmarks are unmistakable. Surface anatomy of the sternocleidomastoid matters because clinicians use it as a living landmark, not a textbook abstraction: the midpoint of its posterior border corresponds to the emergence of the superficial cervical plexus (Erb point), a common site for regional anesthesia and a known zone for iatrogenic sensory nerve injury. The spinal accessory nerve also traverses this region, exiting the sternocleidomastoid to cross the posterior triangle toward trapezius, so neck dissection can leave a patient with scapular winging and shoulder droop if that course is not respected. Torticollis and postural imbalance often present here.
