Description
Running from the posterior pelvis to the plantar venous plexuses, the deep and superficial veins of both lower limbs are traced in continuity, with the popliteal vein centered in the popliteal fossa as it receives paired venae comitantes from the anterior and posterior tibial arteries and continues proximally as the femoral vein. Laterally and superficially, the small saphenous vein ascends behind the lateral malleolus along the midline of the calf to pierce the deep fascia and terminate in the popliteal vein, while the great saphenous vein, though primarily anteromedial, is also followed as it courses proximally to the femoral vein at the saphenofemoral junction. Proximally, the external and common iliac veins provide the pelvic outflow, and distally the dorsal venous arch and perforating veins link superficial channels to the deep system. Orientation is posterior and bilateral. Clear landmarks. Posterior venous anatomy matters because many clinically relevant problems declare themselves in the calf and popliteal fossa, where deep venous thrombosis most often originates in the soleal and gastrocnemial veins before propagating into the popliteal and femoral veins. This view also clarifies the hemodynamic logic behind varicose disease: incompetent valves in the great or small saphenous systems and their perforators produce reflux from deep to superficial compartments, a pattern that is mapped directly during duplex ultrasound and targeted during endovenous ablation or ligation. Use this plate for gross anatomy and vascular teaching when you need a single, posterior-oriented reference for superficial versus deep venous pathways, perforator connections, and the anatomic context of the popliteal drainage.
