Posterior to the male knee, the popliteal vein occupies the midline of the popliteal fossa, lying deep to the small saphenous vein as it pierces the deep fascia to drain into the popliteal vessel. Proximally the popliteal vein continues as the femoral vein after passing through the adductor hiatus, while distally it receives paired venae comitantes from the anterior and posterior tibial veins that converge behind the proximal tibia. Superficial tributaries track along the posterior calf and curve laterally and medially toward the midline, forming a dense venous network around the posterior capsule and gastrocnemius region. A posterior perspective matters because many clinically relevant venous problems concentrate behind the knee. The popliteal vein is a common site for deep vein thrombosis, and this view clarifies how thrombus can extend proximally into the femoral vein or propagate distally into the calf veins, a pattern that influences duplex ultrasound interrogation and anticoagulation decisions. It also supports procedural planning: endovenous treatment of small saphenous vein reflux, cannulation for venous access, and surgical orientation during posterior approaches to the knee all hinge on appreciating the relationship between superficial and deep venous channels at the saphenopopliteal junction.
