Description
Posterior to the heart and main pulmonary vessels, the trachea descends in the midline and bifurcates at the carina into the right and left main bronchi, with tracheobronchial glands clustered along the posterior tracheal wall and around the proximal bronchi at the hilum. Pulmonary arteries course with the bronchi toward each lung, while pulmonary veins return toward the left atrium in a slightly more anterior and inferior plane, creating a layered relationship that reads cleanly from the back. Green autonomic nerves track along the trachea and bronchial tree toward the pulmonary plexuses. Orientation is unmistakably posterior. That posterior emphasis matters because the largest concentration of submucosal seromucous glands lies in the trachea and proximal bronchi, the same segments targeted by airway suctioning, bronchoscopy, and many tracheal resections and reconstructions where gland-rich mucosa influences secretion burden and healing. It also clarifies why chronic bronchitis and other mucus hypersecretory states manifest most in the larger airways, and why impaired mucociliary clearance can plug segmental bronchi even when distal alveolar architecture remains intact.
