Description
Viewed from above, the pia mater is shown tightly investing the convexities of the left and right cerebral hemispheres, separated along the midline by the longitudinal fissure. Fine pial reflections follow the gyri and descend into sulci, with the central sulcus called out by its depth and position between the precentral and postcentral gyri. Along the superior midline, the superior sagittal sinus is represented with adjacent bridging veins approaching it from the cortical surface, while superficial arterial channels course over the pia within the subarachnoid space. That adherence to the cortical contour is the teaching point. Pia stays bound to the brain surface while arachnoid trabeculae bridge over sulci, creating cisternal spaces and defining where subarachnoid blood tracks after aneurysmal rupture, often layering within sulci rather than stripping directly off cortex. The superior midline venous anatomy matters in trauma and neurosurgery: tearing of bridging veins into the superior sagittal sinus produces subdural hematoma, and parasagittal approaches must respect cortical draining veins to avoid venous infarction.
