Description
Positioned at the thoracolumbar junction, the T12 vertebra is rendered from a superior perspective, with the vertebral body anterior and the vertebral foramen centered posterior to it. Flanking the foramen, the pedicles and laminae form the vertebral arch, while the transverse processes project laterally and the base of the spinous process lies posterior in the midline. Superior articular facets sit posterolaterally and retain a thoracic-style, relatively coronal orientation, in contrast to the more sagittal lumbar facets seen just inferior at L1. Costal facet morphology at T12, often reduced and variable, can be appreciated in relation to the transverse process region. T12 matters because it marks the functional transition from rib-bearing thoracic spine to the mobile lumbar segment, where changes in facet orientation and vertebral geometry alter load transfer and motion. That junction is a common site for compression and burst fractures in axial load trauma, and small differences in T12 morphology can influence how fracture lines propagate into the posterior elements and canal.
