Description
Seen from a superior craniotomy perspective, the superior sagittal sinus courses along the midline in the attached margin of the falx cerebri and widens posteriorly to the confluence of sinuses (confluens sinuum) near the internal occipital protuberance. From this junction, the paired transverse sinuses sweep laterally along the occipital bone before curving inferiorly as the sigmoid sinuses toward the jugular foramina and internal jugular veins. More anterior and lateral, the cavernous sinuses flank the sella turcica, receiving venous inflow from the superior and inferior ophthalmic veins and draining posteriorly via the superior and inferior petrosal sinuses. A superior view like this clarifies why venous sinus thrombosis often announces itself with headache, papilledema, or focal deficits: the dural venous sinuses are rigid channels, so obstruction of the superior sagittal sinus can quickly raise intracranial pressure and impair cortical vein outflow. The cavernous sinus region is the other high yield corridor, where facial and orbital infections can spread through valveless veins and threaten cranial nerves III, IV, V1, V2, and VI, producing ophthalmoplegia, chemosis, and sensory loss. Surgical planning depends on these landmarks. So does interpretation of MR venography and CT venography. Use this artwork for teaching dural venous sinus anatomy in neuroanatomy, head and neck, or neurosurgery courses, and for illustrating clinical reviews on venous sinus thrombosis, cavernous sinus syndrome, and posterior fossa approaches that place the transverse and sigmoid sinuses at risk.
