Description
Inferior exposure of the male skull base reveals the common carotid arteries ascending in the neck and bifurcating into the external carotid artery and internal carotid artery, with the petrous and cavernous segments of the internal carotid coursing anteromedially toward the sella region. Posteriorly, the vertebral arteries enter through the foramen magnum to unite as the basilar artery on the clivus, then divide superiorly into the posterior cerebral arteries; anteriorly, the anterior cerebral and middle cerebral arteries arise from the supraclinoid internal carotid and contribute to the arterial circle (circle of Willis) around the optic chiasm region. Arterial pathways track the foramina and grooves, including the carotid canal, foramen lacerum region, jugular foramen, and hypoglossal canal, with venous channels and dural venous sinuses indicated in blue alongside the arterial network. Spatial relationships are clear. Midline landmarks anchor orientation. Skull base angiostructure matters because clinical neurovascular problems often localize to these bony corridors and junctions, where even small anatomic variants change risk and surgical exposure. Aneurysms at the anterior communicating artery, posterior communicating artery, or basilar tip sit exactly where the arterial circle concentrates flow, and this model-like perspective helps explain patterns of subarachnoid hemorrhage and the distribution of ischemia when an internal carotid or vertebrobasilar stenosis limits collateral circulation. Endoscopic endonasal and transcranial approaches also rely on knowing how the internal carotid relates to the sphenoid and clival midline. These are not forgiving relationships.
