Description
Arising in the neck and ascending toward the cranial base, the common carotid arteries divide into internal and external carotid arteries at the carotid bifurcation, positioned anterolateral to the cervical vertebral column and deep to the mandibular angle. Superiorly, the internal carotid courses more posterior and medial as it approaches the carotid canal, while the external carotid remains more anterior and lateral, giving off cervical and facial branches as it climbs. A transparent skull and neck outline allows the arterial tree to be read against deeper landmarks such as the pharyngeal wall and upper cervical vertebrae. Small-caliber arterioles radiate distally into the scalp and facial territories. Carotid morphology matters because this is where clinicians localize stroke mechanisms and surgeons plan access. The relationship of the internal carotid artery to the pharynx and prevertebral region underlies approaches to parapharyngeal space masses and helps explain why tortuosity or a medially displaced internal carotid can become a hazard during tonsillectomy or transoral procedures.
