Description
Seen from a posterior perspective in a transparent adult male, the paired vertebral arteries ascend in the deep neck, passing superiorly through the transverse foramina of the cervical vertebrae from C6 toward C1 before curving medially to enter the foramen magnum. More laterally, the internal jugular veins descend within the carotid sheaths, while the common and internal carotid arteries are inferred anterolaterally as they course toward the skull base, with occipital and posterior auricular arterial branches typically visible over the posterior scalp. Venous channels over the posterior cranium and upper neck frame the arterial pathways. The bony contours of the occiput, cervical spine, and mastoid region remain faintly legible beneath the vascular overlay. Posterior cervical vasculature matters because the vertebral artery’s serpentine course makes it vulnerable at predictable choke points, at the C1 to C2 segment during rotation and extension, and at the transverse foramina in cervical spondylosis, where irritation or dissection can present with posterior circulation symptoms. This view also clarifies why posterior approaches to the upper cervical spine and instrumentation around the lateral mass and pedicle demand constant awareness of the artery’s proximity to the vertebral groove of the atlas and the foramen transversarium. Few images teach that relationship cleanly. This one does. Use it in gross anatomy and neuroanatomy teaching to contrast vertebrobasilar supply with carotid circulation, and in surgical education materials covering posterior cervical decompression, C1 lateral mass fixation, or endovascular discussions of vertebral artery dissection.
