Description
Posterior to the femur, the semitendinosus is presented as a superficial medial hamstring muscle running from the ischial tuberosity to the proximal medial tibia, its long distal tendon narrowing as it approaches the knee. Medial to it, the gracilis descends along the medial thigh, while lateral and slightly deeper hamstring contours suggest the adjacent biceps femoris and semimembranosus. Superiorly, gluteus maximus caps the posterior pelvis and overlaps the proximal hamstring origin region, and inferiorly the hamstrings blend into the popliteal fossa area above the gastrocnemius heads. Two posterior thigh muscles are color-emphasized to separate the target anatomy from surrounding bulk. Clear separation matters. This posterior perspective is the one clinicians think in when tracing posterior thigh pain and planning graft harvest. Semitendinosus autograft, often paired with gracilis, remains a common choice for anterior cruciate ligament reconstruction, and the key teaching point is how the tendons converge distally at the pes anserinus while the muscle belly stays medial and superficial relative to deeper semimembranosus. Proximally, the shared hamstring origin at the ischial tuberosity is a frequent site of avulsion injury in sprinting athletes, and this angle helps clarify why proximal tears can hide under gluteus maximus and still produce extensive ecchymosis down the posterior thigh. Sports medicine lectures, orthopedic operative manuals, and physical therapy curricula can use this artwork to orient the hamstring compartment before discussing palpation landmarks, strain grading, or postoperative ACL rehab precautions.
