Description
Centered on the posteromedial thigh, the semimembranosus runs deep and slightly medial to the semitendinosus, descending from the ischial tuberosity toward the proximal tibia. Laterally, the long head of biceps femoris forms the outer hamstring contour, while gluteus maximus sits superiorly over the posterior hip and upper thigh. Distally, the medial hamstrings frame the superior border of the popliteal fossa, with popliteus emerging deep at the posterior knee as the hamstring tendons diverge toward their insertions. A posterior view like this matters when you need to teach hamstring compartment anatomy and the landmarks that guide diagnosis of posteromedial knee pain. Semimembranosus strain or tendinopathy often localizes near its distal tendon, and its bursa is a common anatomic neighbor of a Baker cyst, which typically tracks between the semimembranosus tendon and the medial head of gastrocnemius. The broad distal expansion of semimembranosus also contributes to the oblique popliteal ligament, a detail that clarifies why injuries near the posteromedial capsule can feel like hamstring pathology and why surgical approaches to the posterior knee respect these layers. A clean landmark. The iliotibial band on the lateral thigh provides a useful counterpoint for orientation and for discussing lateral knee pain patterns relative to the biceps femoris.
