Description
Arising on the posterior cranial fossa, the sigmoid sinus is shown from an inferior (basal) perspective as it curves in an S-shaped groove along the mastoid part of the temporal bone and approaches the jugular foramen. At its inferomedial end, the sinus narrows to become the superior bulb of the internal jugular vein, lying lateral to the medulla and inferior to the cerebellar hemispheres. Nearby basal landmarks expected in this view include the transverse sinus transition posteriorly, the cerebellum and brainstem centrally, and multiple cranial nerves coursing anteroinferiorly from the pontomedullary junction toward their respective foramina. Blue venous channels contrast with yellow cranial nerves against the reddish cerebral and cerebellar cortex. An inferior view of the sigmoid sinus matters because it is the venous outflow bottleneck at the skull base, and its relationship to the jugular foramen and petrous temporal bone dictates both symptom patterns and operative risk. Mastoiditis and otitis media can spread to produce lateral sinus (sigmoid) thrombosis, and this angle helps explain why headache, papilledema, and septic embolic phenomena may coexist with ear symptoms. Otologic and skull base procedures, including mastoidectomy and approaches near the endolymphatic sac, must respect the thin bony plate over the sinus and the variable height of the jugular bulb. Bleeding here is unforgiving. Use this asset in neuroanatomy teaching on dural venous sinuses, in radiology correlation for CT/MR venography of jugular bulb variants and venous sinus thrombosis, and in operative anatomy figures for lateral skull base and otology texts where the sigmoid-jugular transition is a key landmark.
