Anteriorly oriented over the cranial base, face, and upper thorax, the anterior jugular vein is traced as a paired superficial channel descending close to the midline of the neck, typically superficial to the infrahyoid strap muscles and anterior to the carotid sheath. Lateral to it, the external jugular vein runs over the sternocleidomastoid, while the internal jugular vein lies deeper within the sheath alongside the common carotid artery and vagus nerve. Superiorly, facial venous tributaries are suggested around the orbit and nasal region, and inferiorly the anterior jugular vein courses toward the jugular venous arch above the suprasternal (jugular) notch and on to the subclavian or brachiocephalic venous system near the clavicle. Bony landmarks, including the mandible and clavicle, anchor the surface anatomy. A midline, superficial venous pathway like the anterior jugular vein matters when teaching venous drainage patterns that do not follow the carotid arteries, and when explaining why venous access or bleeding can surprise you during low anterior neck dissection. Tracheostomy, thyroid surgery, and cricothyrotomy all intersect the same corridor where the jugular venous arch may cross, and anatomical variation in this venous plexus is a common reason for troublesome superficial hemorrhage or difficult hemostasis.
