Description
Originating at the medial canthus, the angular vein is traced as it forms from the confluence of the supraorbital and frontal veins near the inner orbit, then descends inferiorly along the lateral margin of the nose. An anterior perspective places it superficial to the maxilla and nasal bones, running medial to the orbit and lateral to the nasal bridge before continuing as the facial vein. Adjacent red arterial channels are expected to include the facial artery and its terminal angular artery, creating the familiar paired course of vessels along the nasolabial region. Bony landmarks such as the piriform aperture, nasal cavity, and maxillary dentition frame the vessel’s path. Clinically, the angular vein matters because it participates in valveless venous connections between the face and the orbit, communicating with the superior and inferior ophthalmic veins and, by extension, the cavernous sinus. This is the anatomical basis for cavernous sinus thrombosis arising from infections in the “danger triangle” of the face, and it explains why periorbital swelling and ophthalmoplegia can follow what begins as a seemingly local nasal or upper lip infection. Small vessel position at the medial canthus is also a practical consideration during lacrimal procedures and periorbital injections. Surface anatomy with consequences.
