Description
Arising from the venous arcades along the descending colon, the left colic vein ascends in the left mesocolon and tracks superiorly toward the inferior mesenteric vein, running close and often parallel to the left colic artery. Medial to the colon, its tributaries communicate with marginal veins near the splenic flexure and with sigmoid veins inferiorly, while the vessel itself lies anterior to the retroperitoneum and lateral to the abdominal aorta. Surrounding context in this anterior abdominal view logically includes the inferior mesenteric artery and vein, mesenteric fat, and adjacent autonomic nerves and lymphatics accompanying the vascular pedicle. Orientation is clear. This anterior perspective matters because the left colic vein is a consistent landmark in left-sided colorectal surgery, where the inferior mesenteric vein may be ligated near the lower border of the pancreas to gain length for a tension-free anastomosis. During mobilization of the splenic flexure, traction on the mesocolon can avulse small venous tributaries near the left colic vein, a common source of troublesome bleeding when the plane between mesocolon and Gerota fascia is not clean. The parallel course with the left colic artery also helps teach the concept of paired mesenteric vessels and their relationship to lymph nodes along the inferior mesenteric pedicle.
